We have 3 part time positions available, to be based at Withybush Hospital, Haverfordwest. The proposal is to extend the provision of the Hospital Discharge team to develop a seven day service and extension of the working day to cover a period of the evening. It is proposed that the service will be provided between Monday to Friday 8.00am – 8.00pm (existing team) 5.00pm - 9.00pm Saturday & Sunday 10.00am – 6.00pm It is proposed that this will be staffed by 3 Assessor Care Co-coordinators Post 1 - Monday to Friday 5pm to 9pm (20 hours) Post 2 - Friday, Saturday, Sunday 10am to 6pm (22 ½ hours) Post 3 - Saturday, Sunday, Monday (22 ½ hours) Please indicate clearly in the skills and experience section of your application form which post (s) you wish to be considered for Your role will be to undertake assessments, care planning and reviews which are needs-led, and target resources to those who are in greatest need. You will assess and/or co-ordinate the assessments of the strengths and needs of users, develop and cost packages of care which are flexible, cost effective and responsive to the changing needs of the individual. You will promote independence and self determination and contribute to the management of the community care budget, maintaining expenditure within set budget limits. In addition you will be expected to contribute towards the team’s performance management by providing regular performance information, and actively work in partnership with colleagues from other agencies. You will review and/or co-ordinate the care packages on a regular basis and facilitate changes as necessary, identifying service deficiencies and reporting to the Customer Services Manager You will be educated to secondary level education, and ideally hold a HNC (or equivalent) qualification in a relevant discipline. You will have considerable experience of assessing the needs of the elderly and/or physically and/or sensory disabled, together with experience of working with carers. Knowledge of NHS & Community Care Act 1990 is essential as is a valid driver’s licence. Job Type: Permanent Location: Sweden - Jφnkφping County
